REGISTRATION FORM HYPERBARIC TECHNICIANS & NURSES ASSOCIATION 33RD ANNUAL SCIENTIFIC MEETING OF DIVING AND HYPERBARIC MEDICINE

Fiona Stanley Hospital Hyperbaric Facility, Fremantle, WA

28th October - 2 x Guided Tours and HTNA AGM

Tours - pick up from Hougoumont Hotel Fremantle 1pm

AGM - directly after tours at the Hougoumont Hotel Fremantle 4-6pm

29th – 30th October - HTNA ASM, Maritime Museum Fremantle 

29th October - Welcome Drinks Maritime Museum / Sundowner Deck 6-8pm

30th October - Gala Dinner, Bathers Beach House Fremantle

Full Conference Package includes:

• 2 x tours & AGM

• 2 x full conference days (including morning & afternoon teas & lunches)
  
  
• 1 x ticket to the conference welcome drinks Thursday evening 28th October (18:00 - 20:00)
  
  
• 1x ticket to the HTNA gala dinner Friday evening 30th October

Payment Details:

• HTNA accepts payments via Paypal and Direct Deposit / Bank Transfer.
  
  
• All Paypal funds are sent to info@htna.com.au (Please note “Hyperbaric Technicians & Nurses Association” will appear on your bank statement)
  HTNA incur a fee of 2.9% to process PayPal payments, please use direct deposit / transfer if available

    These details are also on the invoice sent to your registration email.

• HTNA are unable to process Visa / Mastercard / Dinners or Amex directly.
  
  
• Payment must be made in Australian Dollars. Registration fees will apply according to the date of receipt of the payment.

Cancellation, Substitution and Refund Policy:

• Requests for cancellation/replacement must be made in writing to the Treasurer via conference@htna.com.au
  
  
  • For cancellations received on/before 2nd October 2026: 60% of registration fee will be refunded. All refunds will be made after the event.
    
    
  • For cancellations received after 16th October 2026: 40% refund.
    
    
  • For cancellations received after 28th October 2026: No refund.
    
    
• Applicants who are unable to attend the conference may send an alternate. Substitution requests must be submitted in writing to the HTNA Secretary, conference@htna.com.au

Liability and Disclaimer:

• Whilst every attempt will be made to ensure that all aspects of the Conference mentioned herein will take place as scheduled, the Conference organizers & HTNA Executive reserve the prerogative to make final changes should the need arise. The applicant acknowledges that he/she has no right to lodge complaints against the Conference organizers should the holding of event be hindered or prevented by unexpected political or economic events or by force majeure, nonappearance of speakers or other reasons which necessitate program changes. With registration, the applicant accepts this provision.
